I just wanted to say I had ordered this a month or so ago when I stumbled across it and it’s great! My daughters doctor and I went through it and filled out the previous record information and I’ve kept it handy.
If you don’t want your name out there to advertisers and are afraid of ordering it, you can do some of this broken down in a notebook – but really, they have such great checklists, it’s quite amazing..
